Please complete this form in its entirety. A change of degree requires a student file review, and you will be notified via DU email if there are any questions or concerns.

If you would like to add a dual degree (i.e. adding an MS-Finance to an MBA) you will need to fill out a separate admissions application at http://www.daniels.du.edu/Apply_Now.aspx.
